Summary
This pilot study tests whether having a pharmacist oversee continuous glucose monitoring (using a FreeStyle Libre device) can improve blood sugar control and health behaviors in people with type 2 diabetes. Forty participants will use the monitor for 12 weeks, and their results will be compared to a historical group that only used standard finger-stick testing. The study aims to see changes in A1c levels and other glucose measures.
