Summary

This study looks at whether adding a clinical pharmacist to community mental health teams (called FACT teams) helps people with severe mental illness and substance use disorder feel better and have a better quality of life. About 160 adults will take part. The pharmacist will review medications, talk with patients, and work with the team to solve medication problems. The goal is to see if this extra support makes a real difference.
