Summary
This study is testing a new 6-week personalized care program designed to improve how healthcare professionals coordinate care for people with chronic pain. It will involve 66 patients across three primary care centers in Spain to see if the program is practical and helpful. The goal is to reduce the confusion and burden patients often face by formally organizing the care team around each person's specific needs.
