This study looks at whether a "Patient Navigation 2.0 Checklist" can help Chinese adults in Chicago get recommended cancer screenings and address social needs like housing or food. About 600 people from Chinatown-area zip codes are taking part. The goal is to see if this simple tool makes it easier to connect people with the care they need.
