Summary
This study tests a home-based occupational therapy program that uses activity tracking, coaching, and video calls to help stroke survivors improve daily functioning. Researchers will compare this approach to usual care in 171 older adults leaving geriatric rehab. The goal is to see if it boosts independence, self-management, and physical activity over 6 months.
