Heart failure patients get a boost: nurse education may cut readmissions

Summary
This study looks at whether a 30-minute face-to-face lesson with a nurse, using a 'teach-back' method, can help people with chronic heart failure stay stable and take better care of themselves after being discharged from the hospital. About 118 adults will take part, with half getting the extra education and half receiving usual care. The goal is to see if the teaching improves symptoms, medication compliance, and self-care over three to twelve months.
