Nurse coaching boosts heart failure care for patients and families

Summary
This study tests whether special education from nurses can improve the quality of life and self-care for people with chronic heart failure, while also helping their informal caregivers. About 280 patients and their caregivers will take part. The goal is to see if this approach leads to better health outcomes and less burden on caregivers.
