Nurse-Led discharge may boost patient Self-Care

Summary
This study tests whether a nurse-led discharge plan helps patients become more active in managing their own health after leaving the hospital. About 120 adults in Hong Kong will be randomly assigned to receive either the nurse-led plan or usual care. The main goal is to see if the plan improves patient activation, which is a measure of how confident and able a person feels to manage their health.
