Nurse-Led discharge plan aims to boost patient Self-Care

Summary
This study tests whether a nurse-led discharge program helps patients become more active in managing their own health after leaving the hospital. About 120 adults in Hong Kong who are stable enough to go home will take part. Researchers will measure changes in patient activation using a questionnaire at the start and after three months.
