Summary
This study tests whether a nurse-led discharge plan helps patients become more active in managing their own health after leaving the hospital. Researchers will enroll 120 adults in Hong Kong who are stable enough to go home. The main goal is to see if patients feel more confident and capable in handling their health three months after discharge.
