Summary
This 12-month study tests whether a nurse-led smartphone app can help people with type 2 diabetes who use insulin and are on Medicaid better manage their blood sugar. Participants will use the app to track glucose, get education, and receive phone support from nurses. The goal is to see if this approach improves blood sugar control and diabetes knowledge.
