Summary
This study tested whether regular meetings between different types of doctors could work well in a hospital to help people with diabetes and other chronic conditions. 55 patients joined, and researchers tracked how often the meetings happened, if doctors followed the recommendations, and how patients felt. The goal was to see if this team approach is practical, not to directly treat or cure the disease.
