Summary
This study tested a 12-month program that provides remote support to older adults with type 2 diabetes. Participants received personalized goal setting, phone-based care management, and help connecting to community resources. The goal was to improve blood sugar control and quality of life while reducing dangerous side effects like low blood sugar. The study included 512 adults aged 65 and older living in the community.
