Summary
This study tests whether remote doctor visits and home monitoring devices help people with chronic conditions like diabetes and high blood pressure manage their health better. About 40 adults already in a wellness program will use a tablet or phone to track their health and talk to their care team. The goal is to see if this approach improves confidence, medication adherence, and health numbers like blood pressure.
