Summary
This study tests whether a team of clinical pharmacists can improve medication safety for elderly nursing home residents. About 219 residents will have their medications reviewed using a digital tool, with feedback given to their doctors. The goal is to reduce inappropriate prescribing and related harms like falls and hospital visits. The study is low-risk and embedded in routine care.
