Summary
This study tests a program called SPIDER that helps family doctors, nurses, and pharmacists work together to reduce risky medications in older adults (65+) who take 10 or more drugs. The goal is to cut down on prescriptions that may cause harm, like stomach acid reducers and sleep aids. About 104 patients and their care teams are taking part in Toronto, Edmonton, and Montreal to see if this approach improves health and quality of life.
