Inclusion Criteria: * Adults (18+) * Able to provide informed consent. * Proficient in English to answer questions about your health * Are a UK resident * Have an email address and phone number * Have used or tried to use healthcare services at least 5 times in the past two years for any kind of health issue. * Have a diagnosis of a long-term condition (e.g., asthma, diabetes, heart disease, IBS, depression) or have experienced a persistent symptom for a period of 6 months Exclusion Criteria: * Lacking capacity to give informed consent. * Under 18 * Not based in the UK * Have not used or tried to use healthcare services at least 5 times in the past two years
