Summary
This study tests a new way to manage care for older adults (ages 60-74) who have both high blood pressure and type 2 diabetes in rural China. About 960 people from community health centers will either receive the new integrated care package or continue with standard care. The goal is to see if the new approach helps more people control their blood pressure and blood sugar levels over six months.
