Summary
This study looks at whether a new tool can help young adults (ages 18-30) with type 1 diabetes and their doctors work together better to make care plans that fit daily life. Researchers will compare usual doctor visits to visits where the tool is used. Participants will have their visit observed, answer questions, and talk about their experience. The goal is to see if the tool improves teamwork and makes care more personal.
