Summary
This study is testing a digital tool to help young people (ages 14–20) who have had or are waiting for a liver transplant as they move from pediatric to adult medical care. The tool is designed to help patients and their families manage their health remotely, reducing the need for travel. Researchers will also look at what helps or hurts the emotional well-being of these patients during this challenging time.
