New app aims to ease tough transition for young liver transplant patients

Summary
This study is testing a digital tool to help young people (ages 14-20) who have had or are waiting for a liver transplant as they move from pediatric to adult medical care. The tool is designed to help patients and their families manage treatment remotely and improve medication adherence. Researchers will also look at what factors affect the patients' mental well-being during this transition. The goal is to better support these patients without replacing their regular doctor visits.
