New pacing technique aims to outperform standard therapy for heart failure
NCT ID NCT07814612
First seen Sep 11, 2026 · Last updated Sep 11, 2026
Summary
Researchers are testing whether a pacing technique called left bundle branch area pacing, guided by detailed imaging and electrical data, can improve outcomes for people with heart failure and left bundle branch block. The trial enrolls 100 adults with non-ischemic cardiomyopathy and reduced heart function who have already taken heart failure medications for at least three months. Participants are randomly assigned to receive either the new pacing approach or standard biventricular pacing, and researchers track deaths and heart failure hospitalizations over at least a year.
What this could mean
Our plain-language read of the trial. This is informational only, not medical advice or a prediction.
- Active substance
- left bundle branch area pacing guided by multimodal data
- What this could lead to
- If it works, this could give heart failure patients with left bundle branch block a pacing option that uses the heart's own conduction system and may work better than standard biventricular pacing.
- What could go wrong
- The trial is small and early, so it may not show a clear advantage over standard pacing. The procedure can be technically difficult, and some patients may not improve even with the new approach.

Inclusion Criteria: * Age 18-80 years; * Sinus rhythm, complete left bundle branch block (LBBB) fulfilling the Strauss criteria (QRS duration \>140 ms in men or \>130 ms in women); * Non-ischemic cardiomyopathy, left ventricular ejection fraction (LVEF) ≤40%, and New York Heart Association (NYHA) functional class II-IV; * Signed informed consent. * All enrolled patients received guideline-recommended pharmacological therapy for heart failure for at least 3 months. Exclusion Criteria: * Coronary CT-confirmed ischemic cardiomyopathy; * wide QRS complex with non-LBBB morphology, including right bundle branch block and nonspecific intraventricular conduction block; * persistent atrial fibrillation; * pacemaker replacement without reimplantation of ventricular leads; * patients with special conditions, such as pregnancy, gestation, and advanced malignancy. * All patients underwent detailed recording of electrophysiological pacing parameters intraoperatively and postoperatively, and monitoring for surgical complications.
