Summary
This study aims to see if a patient-centered, team-based care model supported by technology can improve blood sugar control in adults with type 2 diabetes. About 120,000 participants from 16 health centers will be involved over up to 3 years. The main focus is reducing the number of people with very high blood sugar levels (A1c > 9%).
