Summary
This study looks at a new way of caring for Medicare patients who are at high risk of being hospitalized. Instead of seeing different doctors in the hospital and clinic, each patient gets one doctor who manages their care in both places. The goal is to see if this approach improves patient satisfaction and lowers overall healthcare costs. About 2008 patients are taking part.
