Summary
This study tests a new way of caring for Medicare patients who are at high risk of being hospitalized. Instead of seeing different doctors in the hospital and clinic, each patient gets one doctor who cares for them in both places. The goal is to improve patient satisfaction and lower overall healthcare costs. About 2008 patients who were hospitalized at least once in the past year are taking part.
