Can a custom breathing workout help patients leave the ventilator faster?
NCT ID NCT07761468
First seen Aug 12, 2026 · Last updated Aug 13, 2026 · Updated 1 time
Summary
This trial tests whether a specific daily 'dose' of inspiratory muscle training can strengthen the breathing muscles of adults on mechanical ventilation in the ICU. Patients are assigned to one of three training intensities, calculated using a new formula called the Total Inspiratory Work Unit (UTIT), and adjusted daily based on their breathing strength. The goal is to see which dose best improves muscle strength and reduces the total time spent on the ventilator.
What this could mean
Our plain-language read of the trial. This is informational only, not medical advice or a prediction.
- Active substance
- Inspiratory muscle training using a POWERbreathe K5 device, with daily dose calculated by the Total Inspiratory Work Unit (UTIT) formula.
- What this could lead to
- If successful, this could establish a standardized, optimal dose of breathing-muscle training for ICU patients, potentially shortening time on ventilators and speeding recovery.
- What could go wrong
- This is a small, early-phase trial (60 patients) testing three doses, so results may not apply broadly. Training may not improve outcomes for all patients, and there is a risk of muscle fatigue or discomfort.

Inclusion Criteria: * Adult patients (\>18 years of age) admitted to the Intensive Care Unit (ICU). * Invasive mechanical ventilation support for more than 48-72 hours. * Hemodynamic stability (defined as absence of vasopressors or receiving stable doses). * Adequate oxygenation and ventilatory parameters: PEEP \< 10 cmH2O, FiO2 \< 0.6, and PaO2/FiO2 ratio \> 150-200. * Ability to cooperate with instructions or absence of deep sedation. Exclusion Criteria: * Pre-existing neuromuscular pathology directly affecting respiratory muscles. * Severe thoracic trauma or recent thoracic surgery. * Unresolved pneumothorax or bronchopleural fistula. * Severe hemodynamic instability or severe cardiac arrhythmias. * Level of sedation that prevents active participation in sessions. * Pregnancy
