Summary
This study tests two tools to help Black patients with advanced kidney disease get better care and delay the need for dialysis. One tool helps doctors identify high-risk patients, and the other provides health coaching. The goal is to see if these tools can help patients avoid starting dialysis with a temporary catheter and live longer without needing dialysis.
