New program aims to keep young cancer survivors from falling through healthcare cracks

Summary
This study tests whether a patient navigation program helps young adult survivors of childhood cancer successfully transition from pediatric to adult-focused healthcare. About 190 participants aged 20-29 will be randomly assigned to receive either the navigation program or standard care. The goal is to see if the program increases the number of survivors who complete a proper adult survivorship care visit and reduces unmet social needs like housing or food insecurity.
