Helping survivors plan their future care: new study tests support tools

Summary
This study is testing two support tools to help people who have survived a blood cancer transplant plan for their future medical care. It will see if using a special website, meeting with a health coach, or both helps patients have important conversations about their care wishes and complete legal documents. The goal is to improve the quality of life and care for survivors by ensuring their preferences are known and respected.
