Summary
This study tests a questionnaire called AT HARM 10 to help pharmacists identify when a patient's emergency visit is caused by their medications. Researchers will enroll 100 adults over 65 in the emergency department to see how well the tool works. The goal is to improve medication management and reduce future hospital visits.
