Summary
This study aims to help people with uncontrolled high blood pressure (above 150/95) get their numbers down. It uses a team of doctors, pharmacists, and nurses who work with patients to adjust medications based on home blood pressure readings. About 1,520 adults aged 18-85 will take part across 56 primary care clinics in Ohio. The goal is to see if this approach can lower blood pressure safely and effectively over 12 to 24 months.
