Summary
This study aims to help people with uncontrolled high blood pressure (above 150/95) get their numbers down. It will involve about 1,520 adults across 56 primary care clinics in Ohio. The approach uses home blood pressure readings and a care team that includes pharmacists and advanced practice providers to adjust medications as needed. The goal is to lower systolic blood pressure by an average of 18 mmHg over 12 months.
