Summary
This study is testing a new support program for people with type 2 diabetes who are leaving the hospital. It aims to see if screening for social needs like housing or food insecurity, and then providing a helper (a 'navigator') for 90 days, can reduce the need for a return hospital visit. The study will compare this new program to the usual care patients receive when they go home.
