Can Home-Based nursing care beat traditional facilities for recovery?

Summary
This study is testing whether providing skilled nursing and rehabilitation care at home is as good as or better than sending patients to a nursing facility after a hospital stay. It will enroll 650 patients in Massachusetts who need this type of care and randomly assign them to receive it either at home or in a facility. The main goal is to see which group has fewer hospital readmissions or deaths in the first 30 days, while also tracking recovery, quality of life, and healthcare costs.
