Summary
This study tests a new way for care managers to support people living with HIV. Instead of relying on schedules and judgment alone, care managers get a computer-generated list of patients most likely to need emergency care in the next two weeks. The goal is to reach those patients early, reduce ER visits and hospital stays, and improve viral suppression. About 2,600 participants from New York care agencies are taking part.
