Summary
This study follows 2,000 adults aged 45-75 in Pennsylvania to better predict who is at risk for heart disease and why some groups are more affected than others. About 800 participants at higher risk will be randomly assigned to either usual care or a special lifestyle program to see which works better. The goal is to improve risk assessment and find effective ways to reduce heart problems.
