Summary
This study looked at two different ways to support heart failure patients after they leave the hospital. One group got a phone call from a care coordinator, while the other could request a home visit from a community paramedic who could connect them with a doctor via video. The goal was to see which approach better reduced hospital readmissions and improved quality of life. About 2,000 patients from New York City hospitals took part.
