Summary
This study is testing a new support program designed to improve the health of people with disabilities who also have chronic conditions like diabetes or high blood pressure. The program provides education and support on nutrition, physical activity, and managing health. Researchers will enroll 600 participants in Hong Kong to see if the program helps them live healthier lives.
