Summary
This study looks at whether a telemedicine program can help older adults living in nursing homes after they leave the hospital. About 102 participants will receive remote check-ins from a geriatric care team. The goal is to see if this approach reduces the number of times they need to go back to the hospital and if it is cost-effective.
