Summary
This study looks at whether a geriatric telemanagement program can help nursing home residents who were recently in the hospital avoid going back. About 102 older adults will take part, and researchers will track if the program reduces hospital readmissions by at least 15%. The goal is to see if remote care can improve health outcomes and save costs.
