Summary

This study looks at whether offering financial rewards, along with nurse coaching and home monitoring, helps adults with type 2 diabetes improve their blood sugar control. About 450 participants will be randomly assigned to receive either nurse coaching and home monitoring alone, or the same plus financial incentives. The goal is to see if the incentives lead to better long-term results and if those benefits last after the rewards stop.
