Summary

This study is testing a new support program called the Family Bridge Program. It aims to help low-income families from diverse backgrounds better navigate the hospital system and communicate with their child's medical team. A trained guide provides practical help, communication coaching, and emotional support during a child's hospital stay and after they go home. The goal is to improve the family's experience, reduce stress, and ensure a smoother transition from hospital to home care.
