New program aims to bridge healthcare gaps for Low-Income families

Summary

This study tests a program called the Family Bridge Program (FBP) designed to help low-income families better navigate hospital care and communicate with doctors. About 728 families with children in the hospital will be randomly assigned to receive the program or standard care. The program includes hospital orientation, social needs screening, communication coaching, and a follow-up call after discharge. The goal is to see if it improves families' ability to navigate the healthcare system and eases the transition from hospital to home.
