Summary

This study tests two ways to help homeless people get follow-up care after emergency room visits. Researchers will compare sending a street medicine team to find patients where they live versus giving them a regular clinic appointment. They'll track whether patients actually get care, plus emergency room returns, hospital stays, and health outcomes over three months.
