Summary
This study is testing if sending a street medicine team to find and treat homeless patients after an ER visit works better than just giving them a clinic appointment. It aims to help people get the follow-up care they need, which could prevent future emergency visits and hospital stays. The study will compare two groups: one gets care from a mobile team that meets them where they are, and the other gets a traditional clinic referral.
