Summary
This study looks at whether a telehealth program called ECHO can help healthcare teams better manage patients with diabetes and multiple chronic conditions. About 20 clinic sites in New Mexico will take part, with some teams receiving the training and others not. Researchers will compare patients' blood sugar levels over 18 months to see if the program leads to better control.
