Summary

This study tests whether giving free fruits and vegetables (a "produce prescription") to adults with congestive heart failure who struggle to afford food can reduce emergency visits and hospital stays. About 900 participants will be split into three groups: one gets produce with minimal support, another gets produce plus extra health coaching, and a third gets usual care. The goal is to see if better nutrition and support can improve health and lower medical costs.
