Summary
This study is testing a telepharmacy service for older adults living at home in rural northern Sweden. The service includes medication interviews, structured reviews, and team discussions to find and fix medication-related problems. Researchers will measure changes in medication adherence, quality of life, and beliefs about medicines. The goal is to see if this digital approach can improve medication safety and patient involvement.
