Summary
This study is testing a digital pharmacy service for older adults living at home in rural northern Sweden. The service includes medication interviews, reviews, and team discussions to find and fix medication-related problems. Researchers will measure changes in medication use, quality of life, and beliefs about medicines. About 100 participants who take five or more medications will take part.
