Summary
This study tests a 12-week nurse-led digital education and support program for young adults (ages 18-45) with type 2 diabetes. The program includes lessons, goal-setting tools, reminders, and messaging with a nurse. Researchers will see if it improves self-care, blood sugar levels, and reduces diabetes-related distress compared to usual care plus digital brochures.
