Digital nurses to monitor heart patients remotely

Summary
This study will test a new way to manage heart disease risk using a digital, nurse-led remote monitoring program. About 750 adults with heart disease or related conditions will use the program as part of their regular care. Researchers will check if more patients reach healthy cholesterol and blood pressure targets, and also measure quality of life and satisfaction.
